I give consent for my child to participate in the MEDscienceTECH Visionaries Program’s off-site visit to the Johnson & Johnson Institute in Raynham, Massachusetts on July 16, 2026. Additionally, I give consent for my child to travel by bus provided by the program. I understand that I may be asked to complete additional consent forms required by these sites.

Should my child require immediate first aid or other emergency care for an injury or illness, I hereby consent for my child to receive necessary first aid or other emergency medical treatment in the event I or the persons designated on this form as additional and/or an emergency contact cannot be reached.

I further hereby, on behalf of myself, my child and anyone claiming through myself or my child, do FOREVER RELEASE the President and Fellows of Harvard College, its trustees, officers, members of its governing boards, employees, volunteers, students, agents and assigns (collectively, “Harvard”) from any cause of action, claims, or demands of any nature whatsoever, including but not limited to a claim of negligence which I, my child, or anyone claiming through myself or my child, may now or in the future have against Harvard on account of personal injury, bodily injury, property damage, death or accident of any kind, arising out of or in any way related to my child’s participation in the Program, howsoever the injury is caused.
